Open Enrollment

December 5 – December 20, 2025

If you do not enroll in the Advantage Max 5000 Plan during open enrollment, you must wait until:

  • A qualifying life event occurs, or

  • The next open enrollment period

You may drop the benefit at any time after you have enrolled.

Here are ShiftMed, we offer a variety of healthcare coverage option to take care of you while you take care of patients. After working just one shift, you can access the quality coverage you need and deserve.

Plan offerings include various medical, dental, vision, and short-term disability.

ShiftMed has also partnered with Hooray Health to offer a unique illness and accident plan for all employees! Employees enrolled in this plan have access to $0 telemedicine, the Hooray Health Network of Retail Clinics and Urgent Care Centers for only $25 per visit, accident coverage, Rx Savings, virtual behavioral health visits, and more!


Enrollment Details

Below are enrollment instructions for new hires, rehires, and current ShiftMed healthcare professionals.

New ShiftMed Hires

  • Once you work your first shift, you may enroll in the Advantage Max 5000 Plan by contacting Hooray Health directly at 844-823-6590

  • If you miss open enrollment, you will need to wait for a qualifying life event or the next annual open enrollment period.

ShiftMed Rehires

  • Rehired professionals become eligible for the Advantage Max 5000 Plan after working their first shift.

Current ShiftMed Workers

  • Current workers who are not enrolled in benefits can sign up during open enrollment.

Hooray Health Coverage

Hooray Health offers a unique benefit for ShiftMed employees called the Advantage Max 5000 Plan. While it is an indemnity plan, and not a true medical plan, it offers some incredible coverages that may meet your needs. If you are interested in this plan, you will have the opportunity to enroll during open enrollment or with a qualifying life event for this Hooray Health plan.

    (includes tests, labs, and X-rays)

Fidelity 401(k) Retirement Savings Plan

A 401(k) is an employer-sponsored retirement savings plan that allows you to save money through payroll deductions on a tax-advantaged basis.

Whether you’re new to retirement planning or experienced in managing your savings, Fidelity provides tools, guidance, and resources to help you make confident decisions.

Getting Help with Your 401(k)

If you have questions or need support with your Fidelity 401(k) account, assistance is available.

Contribution Limits for 2026

  • Annual contribution limit: $24,500

  • Additional catch-up contribution (age 50+): $8,000

Eligibility Requirements

The computation period is the 12-month cycle beginning on your hire date and repeats annually until eligibility is met.

Once eligible, your Fidelity account will be created automatically. You will have 30 days to make your contribution elections before an automatic 4% deferral begins.

To participate, you must:

  • Be 21 years or older

  • Work 1,000 hours during the computation period

  • Complete 1 year of service

Opting Out or Changing Contributions

You may opt out of the 401(k) plan or adjust your contribution rate at any time through your Fidelity account. Fidelity representatives can assist if you need help accessing or managing your elections.


Contribution Types

Traditional 401(k)

  • Contributions are made pre-tax.

  • Taxes are deferred until you withdraw funds in retirement.

Roth 401(k)

  • Contributions are made after taxes.

  • Qualified withdrawals are tax-free.

Employer Match

ShiftMed contributes 50% on the first 6% of your per-paycheck contribution.
If you elect a Roth 401(k), company contributions will still be pre-tax.

Vesting Schedule

  • After 1 year of employment: 50% vested

  • After 2 years of employment: 100% vested

Vesting determines how much of the employer match you own if you leave ShiftMed.

Workers’ Compensation

As a ShiftMed W-2 employee, you may be eligible for workers’ compensation benefits if you experience a work-related injury or illness. Because requirements differ by state, it’s important to report incidents promptly and follow state-specific guidelines.

If you experience a work-related injury or illness, complete the following steps:

1. Report Your Injury

  • Report any work-related injury or illness to ShiftMed as soon as possible by calling ShiftMed at 1-800-485-9002 or using the chat feature in your app.

  • If urgent medical care is needed, visit the nearest urgent care or emergency room. Additional treatments must follow your state’s rules.

2. File a Workers’ Compensation Claim

  • After reporting your injury, ShiftMed will provide an incident report for you to complete and return.

  • Once submitted, we will initiate your claim and provide you with the appropriate path for care.

3. Claim Review

  • ShiftMed will forward your claim to our workers’ compensation provider, PMA Companies, who will investigate and approve or deny the claim.
