What is a 401(k) Plan and its Contribution Options?

A 401(k) plan is an employer-sponsored retirement savings plan that allows you to save on a tax-advantaged basis through payroll deductions.

Contribution Limits for 2025:

The IRS limits how much you can contribute to a 401(k) each year. For 2025, the contribution limit is $23,500, with an additional $7,500 allowed for participants aged 50 and above.

Eligibility to Contribute:

You must be over the age of 21 with a minimum of 1,000 hours worked in the computation period and 1 year of service to participate in the 401(k) plan. The computation period is the 12-consecutive-month period beginning with an employee's hire date. The computation period resets each anniversary until requirements have been met. Once requirements for age, hours worked, and service are met, your Fidelity account will be set up. After your account is setup, you'll have 30 days to access and make elections before the automatic deferral of 4% begins.

Opting Out of Contributions:

While the 401(k) plan features automatic enrollment, you have the option to opt out or make changes to your contributions. It's essential to access your account promptly to manage your preferences.

Contribution Options:

There are two options when it comes to contributing to the 401(k) plan:

  1. A Traditional 401(k) - this will save pre-tax income from your paycheck and income taxes are not withheld when contribution is made but will be taken when you withdraw your funds.

  2. A Roth 401(k) - this will save after paying income taxes on your contributions and no taxes will be taken out when you withdraw your funds.

Making Changes to Contributions:

You can make changes to your 401(k) contributions at any time through your Fidelity account. If you encounter any difficulties, Fidelity's representatives are available to assist you.

Employer Match

ShiftMed offers a company match/contribution of 50% on the first 6% of your contribution (per paycheck). If you are enrolled in a Roth 401(k), company contributions will be pre-taxed.

Vesting Schedule

  • After 1 year of employment: 50% of your contributions become vested. This means you gain ownership or rights to half of the contribution amount made by your employer.

  • After 2 years of employment: 100% of your contributions become vested. This means you gain full ownership or rights to the entire contribution amount made by your employer.

Workers Compensation

As a ShiftMed W-2 employee, you may be eligible for workers’ compensation if you suffer from a work-related illness or injury.

Every state has different laws for reporting injuries and filing workers’ compensation claims. Visit the Office of Workers’ Compensation Programs (OWCP) page on the U.S. Department of Labor website to see how your state handles workers’ compensation claims.

Basic Workers Compensation Protocols

Report Your Work Injury

Please contact ShiftMed to report your work-related illness or injury as soon as possible. States have different time limits regarding workers reporting illness or injury to their employers. If your injury requires immediate medical attention, visit the nearest urgent care or emergency room. You’ll need to follow your state’s rules for seeking medical treatment for all other treatments.

File a Workers’ Compensation Claim

When you contact ShiftMed about your work-related illness or injury, we’ll provide you with an Impairment Rating (IR) form that you must complete and return. From there, we’ll open a workers’ compensation claim for you.

Open Workers’ Compensation Claim

ShiftMed will submit your workers’ compensation claim to our workers’ compensation provider, PMA Companies. PMA will investigate the incident before approving or denying your claim.

If you have any questions about your open workers’ compensation claim, you must contact PMA at 888-476-2669. If you need additional support, please contact the ShiftMed Help Line at 800-485-9002.
