Ohio Background Check Options

How do I get a background check to be able to work in Ohio?

There are several options available for the Ohio Background Checks

1) You already have a copy of your BCI Background Check that was completed within the past year - Applicants can submit a previous BCI Ohio Background check to us by either emailing the background check to bg@shiftmed.com or by using the Get Help option in the app.

2) You have already completed the BCI Background Check, but do not have a copy on hand, you can complete the request form below to have a copy of your BCI Background check sent to ShiftMed.

You can request a copy of the previous BCI Ohio Background Check by using the following form.

Please note that if the request is made within 30 days of the original background check there is no fee for processing and the request can be faxed to 866-750-0214.

30 days after the original background check the fee is $8 and is payable by money order, certified check, business check or personal check to Treasurer, State of Ohio. No cash will be accepted. The request must be received within 11 months of the original fingerprint submission in order to have time to process before the background check expires.

The request and payment can be mailed to: BCI, PO Box 365, London, OH 43040.

3) You do not have a current BCI Ohio Background Check.

You will need to complete a new background check. Great News! We have partnered with FastFingerprints to help get your background check completed. Please print out and complete the form below. We will cover the full cost of this background check if you use our direct bill form at a FastFingerprints location.

You must schedule an appointment in advance by using the link below. Be sure to select "Direct Bill" for your payment option and take the completed Ohio Background Check Form with you to your appointment.

If you have lived in the state of Ohio for the past five (5) consecutive years, then you will need to complete the BCI Fingerprint Background Check only.


If you do not live in the state of Ohio or have NOT lived in the state of Ohio consecutively for the past five (5) years, then you will need to select A combined BCI and FBI Fingerprint Background Check.

Schedule a Time: Select the date and time that works best for you (Select the arrows to move through the dates to find the one that works best for you.)

Summary of Services: Review information on the screen accuracy - Be sure that the location, date and time are correct

Payment: Select Direct Bill

Payment section

If a disqualifying result is returned you will be Terminated from ShiftMed immediately.

A request for a copy of a background check may only be submitted if the original background check was processed for an Ohio Revised Code reason that allows for updates and if the new background check is needed for an Ohio Revised Code reason that allows for updates. If you are unsure of you are able to request a copy, please contact the Civilian Identification Dept. toll free at 877-224-0043.

The name on the request of copy form must exactly match the name that was submitted with the prior fingerprints.

You may only request a copy of the Ohio BCI background check. The FBI result is not permitted to be sent to any address other than what was requested at the time of the original background check. To obtain a new FBI result, a new FBI background check would have to be submitted.
