Thank you for your interest in ShiftMed. Here is a Checklist of requirements for CNAs that are interested in working through ShiftMed in North Carolina.
Please NOTE that you must have a Professional License through the state of North Carolina.
Required Credentials:
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
2-step PPD, 1-step PPD or TB Blood Test (completed within the last year) or Chest X-Ray (completed within the last 5 years) **You will have more facility options if you have a 2-step PPD vs a 1-step PPD.

COVID-19 Vaccine - All HCPs must have either a COVID-19 Vaccination or must complete a Medical or Religious Exemption form in the app.
Training Manual - complete directly within the app using the tap to sign feature
Company Handbook - complete directly within the app using the tap to sign feature
Get 2 References - please enter mobile numbers for your references

Please note that these are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require:
COVID-19 Test - upload your most current negative COVID-19 test result (Please note that partner facilities with the clipboard symbol in-front of their name in the app - may have COVID-19 testing at their facility)
Health Insurance Card - to have on file for the COVID-19 Vaccine
Flu Shot - there are a few partner facilities in the area that require a flu shot during flu season before claiming a shift
Physical (completed within the last year)
Drug Screen - this is required at some facilities and you will notified when you try to claim a shift if the facility requires one. Forms To Complete:
Complete North Carolina Background Check Authorization - complete directly within the app using the tab to sign feature once you have uploaded your TB Test, and ID.
Items that ShiftMed will Verify:
Name-Based Criminal Record Background Check or Fingerprint-Based Background Check depending on your NC Residency. For applicants that have not been a resident of NC for at least 5 years - a Fingerprint-Based Background Check will be required through the NC Department of Health and Human Services. For applicants that have been a resident of NC for 5+ years - a name-based Criminal Record Background Check will be completed through the NC Department of Health and Human Services.
Active Professional North Carolina CNA License
Clear record on the Office of Inspector General (OIG) Exclusion Check
Upload your credentials in the ShiftMed app.
