Thank you for your interest in ShiftMed. Here is a Checklist of requirements for CNAs, LNAs and Certified Caregiver that are interested in working through ShiftMed in Arizona.
Required Credentials:
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
2-step PPD, 1-step PPD or TB Blood Test (completed within the last year) or Chest X-Ray (completed within the last 5 years) **Disclaimer - a 1-step PPD is the minimum testing requirement, however specific facilities may have additional TB testing requirements at their discretion.

Fingerprint Clearance Card Information - please enter your Fingerprint Clearance Card Number or your Fingerprint Clearance Card Application Number into the app.
Get 2 References - please enter mobile numbers for your references
COVID-19 Vaccine - All HCPs must have either a COVID-19 Vaccination or must complete a Medical or Religious Exemption form in the app.
Training Manual - complete directly within the app using the tap to sign feature
Company Handbook - complete directly within the app using the tap to sign feature

Please note that these are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require:
COVID-19 Test - upload your most current negative COVID-19 test result (Please note that partner facilities with the clipboard symbol in-front of their name in the app - may have COVID-19 testing at their facility)
Health Insurance Card - to have on file for the COVID-19 Vaccine
Flu Shot - there are a few partner facilities in the area that require a flu shot during flu season before claiming a shift
For Certified Caregivers interested in picking up shifts at any Memory Care Facility - Food Handler Card
Physical (completed within the last year)
Items that ShiftMed will Verify:
Verify the Arizona Fingerprint Clearance Card
Active Professional Arizona CNA or LNA License, Active Certified Caregiver Certification
Clear record on the Office of Inspector General (OIG) Exclusion Check
Upload your credentials in the ShiftMed app.
