Thank you for your interest in ShiftMed. Here is a Checklist of requirements for LPNs and RNs that are interested in working through ShiftMed in this area.
Please NOTE that you must have a Professional License through the state of Colorado.
Required Credentials:
Documents that must be uploaded into the ShiftMed App before you can start claiming shift:
1-step PPD, 2-step PPD or TB Blood Test (completed within the last year) or Chest X-Ray (completed within the last 5 years)
ID
Social Security Number - used to be able to process your background check
I-9
CPR
Resume - recommended but not required
COVID-19 Vaccine - All HCPs must have either a COVID-19 Vaccination or must complete a Medical or Religious Exemption form in the app.
Training Manual - complete directly within the app using the tap to sign feature
Company Handbook - complete directly within the app using the tap to sign feature
Additional Credentials:
Please note that these are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require:
COVID-19 Test - upload your most current negative COVID-19 test result (Please note that partner facilities with the clipboard symbol in-front of their name in the app - may have COVID-19 testing at their facility)
Health Insurance Card - to have on file for the COVID-19 Vaccine
Get 2 References - please enter their mobile number into the app
Flu Shot - about 50% of our partner facilities in the area do require a flu shot during flu season
Physical (completed within the last year)
Forms To Complete:
Written Authorization to Request a CAPS Check - please answer all questions to the best of your ability - complete directly within the app using the tab to sign feature
Colorado VECHS Waiver and Notice forms for the background check consent
Items that ShiftMed will Verify:
Active Professional LPN or RN through the state of Colorado
Colorado Background Check
Verification in the CAPS system
Clear record on the Office of Inspector General (OIG) Exclusion Check
Upload your credentials in the ShiftMed app.
