IV Therapy Certificate Checklist
A valid IV Therapy Certification should contain the following information:
Your First and Last Name
Specification between 20 Hour and 30 Hour Certification
Issuing Board for the Certification
Completion Date
Instructor Signature
Adding Your Information to the App
Go to My Credentials and tap IV Therapy Certificate located under either Required, Additional, or Unlock depending on your location.
Choose your Certificate Type and click Next
Take or upload a photo
Save and Submit
