If you are currently enrolled in Dental, Vision, Life, Accidental and/or the Hospital Indemnity plan through Hooray Health your plans will end 10/27/24, you must contact BPA to enroll in new plans through AFLAC. These plans will be effective 10/28/24 and replace your existing Hooray Health coverage.

Hooray Health will still offer one unique plan called the Advantage Max 5000 Plan. For

everyone enrolled into this plan, this coverage is being extended through 12/1. Those enrolled into this plan will also have an opportunity to enroll into coverage starting 12/1during the 2025 open enrollment in November.

Advantage Max Plan Benefits

  • A nationwide network of Urgent Care and Retail Clinic facilities you can visit for only a

  • $25 fee (includes Tests, Labs and X-rays)

  • $0 Virtual Primary Care visits

  • $0 Virtual Behavioral Health visits – up to 3 per year

  • Discounted prescriptions

  • Accident coverage

  • A network of specialty providers

  • Mobile app – and more!

  • NEW! Starting 12/1. overage will be active for monthly increments instead of weekly increments.

  • NEW! Starting 12/1. Benefit starting 12/1, Virtual Behavioral Health included in the

    • Hooray Health plan – 3 visits covered at 100%.

Open Enrollment Details

Starting November 11th – 22nd

New ShiftMed Hires

As a new ShiftMed hire, you can enroll into the Advantage Max 5000 Plan benefit by calling Hooray Health directly at 855-479-4008.

ShiftMed also partners with BPA to provide benefits coaches for new hire enrollment and

open enrollment. If you are enrolling into other benefits plans with a BPA coach, you can

ask to be transferred to Hooray Health as well!

ShiftMed Rehires

You’re eligible to enroll into an Advantage Max 5000 Plan if you are rehired and work your

first shift at ShiftMed!

Current ShiftMed Workers

If you are a current employee with Hooray Health Coverage for dental, vision, life, accident, or hospital indemnity, your coverage will end 10/27 and you can call BPA between 10/8 and 10/25 to enroll into AFLAC coverage for comparable plans.

For everyone enrolled into the Advantage Max 5000 Plan, this coverage is being extended

through 12/1. Those enrolled into this plan will also have an opportunity to enroll into coverage starting 12/1 during the 2025 open enrollment in November.

Qualifying Life Event

If you do not enroll into coverage for the Hooray Health Advantage Max 5000 plan during

open enrollment, you will have to wait until a qualifying life event occurs or annual open

enrollment to enroll. However, you are free to drop the benefit at any time after enrolling.
