How to complete your medical clearance assessment for an N-95 Mask Fit Test

When preparing for an N-95 Mask Fit Test (MFT), which is required at SSM Health and other health system facilities, you must first be cleared to be tested. The medical clearance assesses whether you are at risk of harm from wearing a respirator based on your health history and conditions.

The industry standard form is provided by OSHA and asks various questions of a provider about your fitness to undergo the test (do you have asthma or other respiratory illnesses, etc.). You will need to take a filled-out Respirator Fit Test Form to your Orientation shift.


How to Complete the Medical Clearance Assessment

Once you have signed up, if you are required to get an N-95 Mask Fit test, you will receive an email from out team with an Access code for the assessment.

  1. Enter the access code you received in your email

  2. Input your information
    ​

  3. Sign the disclaimer
    ​

  4. Take the assessment

The online assessment consists of around 35 multi-part questions and will take approximately 5-7 minutes to complete.

Final Steps

Once the medical clearance assessment is cleared, you will receive an email with a printable form. Make sure you print the form and bring it with you for your orientation shift.
