Starting December 5, 2021, the United States instituted a federal mandate requiring healthcare workers to get the COVID-19 vaccine to help protect long-term care residents and patients from contracting COVID and continuing the spread of the virus.
As a ShiftMed healthcare professional, you must show proof of receiving a COVID-19 vaccination by uploading your information into the ShiftMed app. If applicable, you can submit a religious or medical exemption form.
Please note our facility partners have different requirements, and some will not accept religious and medical exemptions. So, if you have an exemption, you won’t be able to claim shifts at these locations.
Vaccination Types
Below is the list of COVID-19 vaccinations we accept and the doses required for full vaccination.

Vaccine Card Information
Go to My Credentials and tap COVID-19 Vaccine Card.
Choose the vaccination type:
For AstraZeneca and Novavax, choose Pfizer dose 1 for the first dose and Pfizer dose 2 for the second.
For Sinovac, choose Moderna dose 1 for the first dose and Moderna dose 2 for the second.
Follow the steps to upload an image of your vaccine card.
Repeat steps 2 and 3 to upload a second dose.
COVID-19 Vaccine Boosters
There’s no booster for individuals who received a 2023-2024 COVID-19 vaccine. If you received your vaccine before 2023, you may have been required to get a booster to be considered fully vaccinated.
If you received a booster, here’s how to upload the information into the ShiftMed app:
Go to My Credentials and tap COVID-19 Vaccine Card.
Choose your booster type
You can upload information for any brand of booster. It doesn’t need to match your initial vaccine brand.
Follow the steps to upload an image of your booster information.
You may need to upload screenshots of the front and back of your card. If so, upload the front image, then select the Add Picture button to upload the back image.
COVID-19 Vaccine Exemptions
You can submit an exemption in the app if you haven’t received a COVID-19 vaccination due to medical concerns or religious reasons.
Please note - Some of our facility partners, no matter the state of operation, will not accept medical or religious exemptions.
Here’s how to submit an exemption in the app:
Go to My Credentials and tap COVID-19 Vaccine Card.
Tap the I HAVE AN EXEMPTION button.
Select YES, MEDICAL, or YES, RELIGIOUS based on your exemption type.
Complete the presented forms and answer all of the questions. We will review the information ASAP.
