Resources to help you locate a clinic for your Physical, TB Test, or Immunization

As part of your onboarding or to unlock specific shifts, you may be required to upload a Physical, TB Test, or Immunizations. While ShiftMed cannot help you pay for these tests, we do have a few resources to help you find a clinic near you.


Physical

An annual physical can be a full physical exam or a statement of health from your doctor clearing you to work.

If you want a blank physical form: click here. Please note that you are NOT required to use this form.


Tuberculosis Test

You may be required to get a 1-step PPD or 2-step PPD, depending on where you want to work. We may be able to accept a TB Blood Test or Chest X-Ray. Check your state's specific credentialing requirements for details.

Immunizations

Depending on what state you live in or what nursing license you hold, you may need to upload multiple Immunization records. Please check your state's specific credentialing requirements for details.

Once you have completed your Physical, TB Test, and/or Immunization, make sure you upload the document(s) in the app so our team can review them.


Drug Screening

ShiftMed will pay for your Drug Screening should you need one. Unfortunately, we cannot accept previously completed Drug Screens through another company, this must be completed through us. If you need a Drug Screen please reach out to us via Chat! Do this by in the app selecting Menu > Help Center > Help Article & Chat > Messages > Send us a Message.
