Thank you for your interest in ShiftMed. Here is a Checklist of requirements for Respiratory Therapists that are interested in working through ShiftMed in this area.
In order to be eligible, an RT must hold a current national credential (CRT and/or RRT) with the National Board for Respiratory Care (NBRC) and a valid State license in ALL states that they are seeking employment in (except Alaska which does not require a state license).
This means you must have a combination of your RT and CRT license or your RT, CRT and RRT in order to be hired.

Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:

1-step PPD, 2-step PPD or TB Blood Test (completed within the last year) or Chest X-Ray (completed within the last 5 years)
ID
I-9
COVID-19 Vaccine - All HCPs must have either a COVID-19 Vaccination or must complete a Medical or Religious Exemption form in the app.
Training Manual - complete directly within the app using the tap to sign feature
Company Handbook - complete directly within the app using the tap to sign feature

Please note that these are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require:
COVID-19 Test - upload your most current negative COVID-19 test result (Please note that partner facilities with the clipboard symbol in-front of their name in the app - may have COVID-19 testing at their facility)
Health Insurance Card - to have on file for the COVID-19 Vaccine
Get 2 References - there are a few partner facilities that require verified references before claiming a shift - please enter mobile numbers for your references
MMR - some of our partner facilities require this credential before claiming a shift
Flu Shot - some of our partner facilities in the area do require a flu shot during flu season
Varicella - some of our partner facilities require this credential before claiming a shift

Completed BID Form - Background Check Consent - please answer all questions to the best of your ability
If you are from out of state or do not have 3 years of residency in the state of Wisconsin - completed the CheckR Email to start a Criminal Background Check - this will be emailed to you once we have completed your Wisconsin Background Check
Items that ShiftMed will Verify:
Wisconsin Criminal Background Check
SAM (Only required for RRTs)
If from out of state or do not have 3 years of residency in the state of Wisconsin - a completed CheckR Criminal Background Check
RRTs & CRTs must have an active WI state RRT / CRT license and also have an active license with National Board for Respiratory Care
Clear record on the Office of Inspector General (OIG) Exclusion Check
Upload your credentials in the ShiftMed app.
