Ohio: Background Check Requirements

Whether you're an STNA, LPN, or RN licensed to practice in Ohio, you must complete one or more background checks based on your resident status. What scenario applies to you?


You Are a Long-time Ohio Resident

If you have lived in Ohio for five consecutive years or more, you only need to complete the BCI background check.

We can accept a prior BCI background check as part of your credentialing process as long as it occurred within the past 12 months. If the background check occurred over 12 months ago, you must complete a new BCI background check.

You have a copy of your BCI background check.

You can send us a copy of your BCI background check for verification by submitting it in this form.

You don't have a copy of your BCI background check.

If you don't have a copy of your BCI background check, you can submit a request form and have a copy faxed to us at 866-750-0214. A request made within 30 days of your background check completion date is free. The name on the request form must match the name you used during the fingerprinting process.

You can request a copy of your BCI background check only if the original one was done for a reason allowed by the Ohio Revised Code that permits updates. If you're unsure, please contact the Civilian Identification Department at 877-224-0043.

Requests made after 30 days require an $8 processing fee that must be submitted along with your request form. You can pay the Treasurer, State of Ohio via money order, certified check, business check, or personal check—cash not accepted.

You Don't Live in Ohio or Are a New Ohio Resident

If you don't live in Ohio or have been an Ohio resident for less than five years, you must complete a BCI and an FBI background check. These background checks are in place to ensure that new Ohio residents and non-residents working in Ohio do not have a criminal record that could pose a risk to the community.

A BCI background check is performed by the state of Ohio, whereas the federal government performs an FBI background check.

You can request a copy of your BCI background check, but you cannot do the same for your FBI background check.

Also, the FBI is not permitted to send your official results to any address other than what was provided at the time of your background check.

How to Schedule Your Appointment

  1. Select "Enter Account Code."

  2. Select "I Have a Code"

  3. If you're an Ohio resident of five (5) consecutive years or more, you need to complete the BCI Fingerprint Background check only. Use the direct bill access code JUI9Y2G, so we can pick up the tab.

  4. If you DO NOT live in Ohio or have NOT lived in the state for the past five (5) years, you must select and complete the combined BCI and FBI fingerprint background check option. Use the direct bill access code JUI9Y3H, so we can pick up the tab.

  5. Enter your zip code.



  6. Select a fingerprinting location that's most convenient for you.

  7. Pick a day and time that fits your schedule.


  8. Fill in your personal information.


  9. Review the information for accuracy.

  10. After completing your fingerprinting, click the button below to send in your receipt.


Background Check Results

If your background check comes back with a disqualifying result, your termination with ShiftMed will be effective immediately.
