First documents you will need to upload prior to completing your profile and uploading state specific requirements:
When signing up to work with ShiftMed in the app you will then be prompted to enter your
Proof of Identification (DL, Photo ID, Passport, etc.)
Social Security Number
Professional License (National license verification + State license verification)
You can upload multiple Professional Licenses and any Specialties if applicable.
Please Note: You must be professionally licensed to practice in this State.

Required Credentials
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
Background Check: If you have a prior BCI Ohio Background check or a receipt for search confirmation please submit to bg@shiftmed.com.
Unlock Credentials
Please note that specific facility credentials could be asked of you when attempting to claim a shift. These are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require. For more information on Facility Specific Credentials, CLICK HERE.
Annual Physical Exam: Completed within the last year.
1-step PPD, 2-step PPD or TB Blood Test: PPD must be completed within the last year. Chest X-Ray must be completed within the last 5 years
COVID-19 Vaccine: Some partner facilities will require either a COVID-19 Vaccination (1 or 2 doses) or a Booster.
MMR: Some of our partner facilities require this credential before claiming a shift.
Varicella: Some of our partner facilities require this credential before claiming a shift.
TDAP: Some of our partner facilities require this credential before claiming a shift.
Flu Shot: Some of our partner facilities require this credential during flu season.
Resume: Some of our partner facilities may require this credential before claiming a shift.
References: Some of our partner facilities require this credential before claiming a shift. Add your references directly in the app.
Specialties: Some of our partner facilities frequently seek clinical support from healthcare professionals with specialized skills. If you're certified in a specialty area of practice, you want this credential on file with us.
CPR: Some of our partner facilities may require this credential before claiming a shift
Items that ShiftMed will Verify
Active Ohio STNA Professional License
Background Check: OH BCII, or OH BCII & FBI Results.
If a prior BCI Criminal Background Check is submitted this will be verified by ShiftMed.
Clear record on the Office of Inspector General (OIG) Exclusion Check
Clear record on the System for Award Management (SAM) Exclusion Check
Special Requirements for Proof of Residency
If you are NOT a resident of Ohio, or have not been a resident of Ohio within the last 5 years, an FBI Background Check is REQUIRED
