If you are a ShiftMed employee and have an expired TB test, you may complete an in-app Annual TB Renewal Agreement.

This agreement, if read and signed, may be able to extend your current TB test. The TB Symptom Screener is a quick, simple form that requires you to answer a few questions and sign to complete.

The agreement contains some information about Tuberculosis and a symptoms checklist. Completing this may allow you to extend your TB test credential on file and continue to work.
