Where to find your State Tax Form.

Not every worker in the United States has to pay state income tax. It depends on the state. As of 2023, Alaska, Florida, Nevada, New Hampshire, South Dakota, Tennessee, Texas, Washington, and Wyoming are the only states that do not levy a state income tax.

If you are required to pay state taxes, you can find the tax form for your state here.

Once you've completed and signed your state tax form, please email it to us at shiftmedpayroll@shiftmed.com.

Due to adjustments and processing times, please allow 2-3 business weeks for your paycheck to reflect your state withholding information.

Am I required to complete a state tax form?

If your state requires you to pay state taxes, you are not required to complete the state tax form. However, the maximum withholding amount set forth by your state will be deducted from your paycheck. You can complete a state tax form at any point during the year.

What if I do not complete a state tax form?

If your state requires you to pay state taxes and you do not complete the tax form for your state, the maximum withholding amount set forth by your state will be deducted from your paycheck.

You can update your Form W-4 (federal tax withholding) and state tax form at any point during the year. Once we have your updated form(s), we make the adjustments in our payroll system, and your updated withholding information will be reflected accordingly within 2-3 business weeks.
