Getting Started: Community Hospital Onboarding Guide for Healthcare Professionals

Everything you need to know about onboarding with Community Hospital!

ShiftMed Requirements

To begin onboarding through ShiftMed, all healthcare professionals will need to submit and complete the following within the ShiftMed app:

  • Proof of Identification (DL, Photo ID, Passport, etc.)

  • Social Security Number

  • Professional License: (6+ months experience)

    • You can upload multiple Professional Licenses and any Specialties, if applicable.

  • Training Manual: Complete directly within the app using the tap to sign feature.

  • Company Handbook: Complete directly within the app using the tap to sign feature.

  • Arbitration Agreement

  • Submit Work History (this is like uploading a resume)


Credentialing Requirements

Candidates must complete the following credentialing expectations:

Background Checks & Screening

  • SAM

  • OIG

  • Background Check (Checkr, State Background Check)

  • Drug Screen (10-Panel)

    • Note: Results with a positive marijuana finding will not be accepted

  • State Registry Check

    • KY, NC


Certifications

  • BLS / ACLS Valid for 2 years

    • Accepted providers: AHA, HSI (BLS/ACLS), ARC


Health Requirements

  • TB Test (all methods accepted)Valid for 1 year

    • Annual agreement renewals allowed

  • Hepatitis B

    • Full vaccine series, positive titer, or declination accepted

  • Varicella

    • Full vaccine series, positive titer, equivocal titer with recent Varicella dose or declination accepted

  • MMR

    • Full vaccine series, positive titer, equivocal titer with recent MMR dose or declination accepted

  • Tdap Vaccine

    • Vaccine, titer or declination accepted

  • COVID-19 Vaccine Card

    • 2 doses, Booster medical or religious declination accepted

  • Flu Shot

    • Flu shot or declination accepted. Typical timeframe for flu season ranges around October — March

  • Annual Physical

    • Completed within the last year


Documentation

  • Resume

    • Must be current and include facilities worked and specialties


Experience Requirements (Verified Specialties)

  • RN (ICU, Med/Surg, SDU)

    • Must have at least 1+ year of recent experience

  • LPN (SDU)

    • Must have at least 1+ year of recent experience

  • CNA

    • Must have acute experience

  • TB Baseline Risk Screening

  • RN Job Description

  • LPN Job Description

  • CNA Job Description

  • Self-Assessment

    • RN, CNA

  • Online Learning Certificate


Orientation Details

  • You must complete the hospital's required online learning and quiz prior to being able to claim orientation shifts.

  • Once all required credentials have been verified and online learning has been completed, you may email LTACRecruiting@shiftmed.com to schedule an orientation shift.

  • You are also required to provide a headshot photo (with plain white background) prior to orientation for hospital access and badging requirements. Failure to do so will result in being unable to access hospital or complete job duties requiring medication access until completed. It may also lead to cancellation of the orientation shift. ShiftMed Attendance Policy & Expectations

  • Clocking in and out:

    • You are required to clock in and out for every shift using the ShiftMed app.

  • Missed Punches:

    • If you are unable to use the app, you must promptly request a missed punch form from the Care Team to correct your time.

  • Cancelling a Shift:

    • If you need to cancel a shift, you must report it in the ShiftMed app.

  • Attendance Expectations:

    • While ShiftMed operates as an “on-demand” or “PRN” platform and may categorize attendance events - such as call-outs, cancellations, and unscheduled shifts - using a weighted system (see more details here), our hospital partners maintain high attendance expectations and may consider any change to a claimed shift to be an attendance violation.

    • Failure to adhere to these procedures, or patterns of excessive lateness or cancellations, may impact your future shift opportunities at all Community facilities.
