ShiftMed Requirements
To begin onboarding through ShiftMed, all healthcare professionals will need to submit and complete the following within the ShiftMed app:
Proof of Identification (DL, Photo ID, Passport, etc.)
Social Security Number
Professional License: (6+ months experience)
You can upload multiple Professional Licenses and any Specialties, if applicable.
Training Manual: Complete directly within the app using the tap to sign feature.
Company Handbook: Complete directly within the app using the tap to sign feature.
Arbitration Agreement
Submit Work History (this is like uploading a resume)
Credentialing Requirements
Candidates must complete the following credentialing expectations:

Registry Check (Louisiana State Adverse Actions List Search)
Drug Screen (10 Panel)
SAM
OIG
Certifications
BLS / ACLS / PALS — Valid for 2 years
Accepted providers: AHA, HSI (BLS/ACLS), ARC
CPR certification requirements will depend on each specialty.
All licenses are required to have BLS
N95 Mask Fit
CPI Blue Card (RN — ED/ER) — Valid for 2 years
NIHSS (Med/Surg, Tele, ICU, ED/ER) — Valid for 1 year
Accepted providers: AHA, Apex Innovations, BlueCloud
Fetal Monitoring Certification (RN — Labor & Delivery)
Fetal Monitoring only
Health Requirements
TB Test (all methods accepted) — Valid for 1 year
Annual agreement renewals allowed
Hepatitis B
Full vaccine series, positive titer, or declination accepted
Varicella
Full vaccine series, positive titer, or equivocal titer with recent Varicella dose
MMR
Full vaccine series, positive titer, or equivocal titer with recent MMR dose
Tdap — Valid for 10 years
Vaccine, Titer
Flu shot — Typical timeframe for flu shot documentation is October – March
Flu shot, Declination
Annual Physical – Valid for 1 Year

Will accept inconsecutive experience that adds up to a year.
Experience must have occurred within the past 5 years.

Orientation Details
Orientation shifts are typically on Mondays.
Once you are ready to claim an orientation shift, the Account Manager will be in touch to schedule a shift based on availability.
What to Expect On-Site
Dress Code Requirements:
Employees are required to wear hospital-designated scrub colors (navy or teal depending on location) with a white undershirt.
Closed-toe athletic or nursing shoes are mandatory.
Attire must be professional and appropriate—prohibited items include clothing with holes, visible undergarments, logos (non-approved), denim, tight or revealing clothing, short skirts, and unapproved headwear.
Personal audio devices such as earbuds and headphones are not permitted.
ShiftMed Attendance Policy & Expectations
Clocking in and out:
You are required to clock in and out for every shift using the ShiftMed app.
Missed Punches:
If you are unable to use the app, you must promptly request a missed punch form from the Care Team to correct your time.
Cancelling a Shift:
If you need to cancel a shift, you must report it in the ShiftMed app.
Attendance Expectations:
While ShiftMed operates as an “on-demand” or “PRN” platform and may categorize attendance events - such as call-outs, cancellations, and unscheduled shifts - using a weighted system (see more details here), our hospital partners maintain high attendance expectations and may consider any change to a claimed shift to be an attendance violation.
Failure to adhere to these procedures, or patterns of excessive lateness or cancellations, may impact your future shift opportunities at all LCMC facilities.
