Form I-9: The Steps for the Authorized Representative

Someone you know (a friend, family member, or acquaintance) is in the process of getting hired by ShiftMed and needs your help to secure official employment.

As an employer, ShiftMed needs to verify each employee's identity and employment eligibility in the United States before they can start work. But because we hire our healthcare professionals remotely, we need an authorized representative to do this verification on our behalf.

The person you know has enlisted you to be the authorized representative in the Form I-9 completion process.

What is the Form I-9?

Before we get into the authorized representative role, here's what you need to know about the I-9. By law, every employee at every company must complete this form as part of the hiring process. The Form I-9 is a government-required document that verifies an individual's identity and authorization to work in the United States.

What is an authorized representative?

Traditionally, a new employee would complete the Form I-9 in person and present the required documentation to their employer. However, because we hire remotely, healthcare professionals must complete the form electronically in our app. The authorized representative completes Section 2 of the Form I-9 for us.

What you need to do as an authorized representative...

Here's how your role as the authorized representative plays out in the I-9 completion process. Let's start with a quick video. If you're still not clear, we've also got step-by-step instructions below.

Step 1

You should have received an email from ShiftMed with the subject line: Help < name > get hired. If you agree to be the authorized representative, you'll use the link in this email to complete and sign the individual's Form I-9—so don't delete the email.

Step 2

Meet with the individual who enlisted you as the authorized representative in person. Refer to the email referenced in Step 1 and select the ˈI Acceptˈ button. After accepting, you'll begin the flow of completing Section 2 of the Form I-9.

Side note: If you receive a 404 Error message after clicking the 'I Accept' button, it's probably because the individual sent you a reminder message. Each time an individual sends a reminder, the authorized representative gets a new email with a new link, which disables all the links in the previous emails. So check to make sure you're using the most current email.

Step 3

Examine the original hard copy document(s) the individual is using to verify their identity and employment authorization. Take note these documents must be what the government considers acceptable documents.

Remember that you're only reviewing this individual's identification and/or employment authorization documents—you're not doing anything with your personal documents.

Step 4

Once you've determined the documents appear genuine and relate to the person, you must compare the information on these documents with the information listed in Section 2 of the Form I-9. (The person you know should already have uploaded copies of the documents in the ShiftMed app prior to meeting with you.)

  • If the individual shows you a List A document, select the List A button on your screen. The document information will auto-populate on the screen. Simply verify the information on the physical document with what is on the screen. If the information doesn't match up, please update accordingly.

  • If the individual shows you List B and C documents, select the List B and C button on your screen. The document information will auto-populate on the screen. Simply verify the information on the physical documents with what is on the screen. If the information doesn't match up, please update it accordingly.

However, if the individual presents document(s) different than what is showing up on your screen, please use the drop-down menu to select the correct document from List A or the correct documents from List B and C. Next, enter the respective document number(s) and expiration date(s). You should not be entering any of your information, only the individual being hired by ShiftMed.

Step 5

After reviewing the document(s), select the check box to indicate that you confirm the accuracy of the information included in List A or Lists B and C.

Step 6

After selecting the check box mentioned in Step 5, you'll come to a screen that includes your pre-filled information. You don't have to populate any information unless the information is incorrect and requires updating.

At the bottom of this screen, you'll select the check box to attest, under penalty of perjury, that the individual (to the best of your knowledge) is authorized to work in the United States. From there, you'll sign off on the form electronically.
