Health Insurance Card Requirement
While we never require health insurance cards to claim shifts at a facility, if you wish to get COVID Tested by the facility you are claiming with, you may submit your health insurance card to help with the costs.
Health Insurance Card Checklist
When adding your information in the app, you must have/include the following:
Health Insurance Providers Name
Your First and Last Name
Note that if you are under someone else's insurance you can still upload the document and we will review it.
Adding Your Information to the App
To add you Health Insurance Card, follow these steps:
Go to My Credentials and tap Health Insurance Card located under either Required, Additional, or Unlock depending on your location.
Select "Next" to take a picture of the front and back of your Insurance Card
Click Submit
