A Mask Fit Test (MFT) is a routine procedure designed to ensure that a tight-fitting respirator (like an N95) forms a proper seal on a healthcare worker's face.
A proper seal is critical because it prevents contaminated air from leaking around the edges of the mask, ensuring the wearer breathes only the filtered air.
What it is?
A medical clearance that assesses if you are at risk of harm from wearing a respirator based on your health history.
Uses an industry-standard OSHA form with questions about respiratory fitness (e.g., asthma).
You MUST bring the filled-out Respirator Fit Test Form to your Orientation Shift.
Without it, you will be unable to proceed with the physical fit test.
Assessment Details
The assessment consists of 35 multi-part questions.
It takes approximately 5-7 minutes to complete.
Who needs it?
Requirement is based on the state and facility you are attempting to claim; some require it, others may not.
How to complete it
Once your orientation shift is scheduled, you will receive an email from ShiftMed with an access code.

Enter the code on the website and complete the evaluation, disclaimer, and assessment.
Once the assessment is completed, you will receive an email with a printable form.
You MUST print this form and bring it to your orientation for the in-person test.
Important: After Orientation, the N-95 mask fit test credential will need to be uploaded into the app to complete the process.
