What credentials do I need to work as a LPN/RN in New Hampshire?

Thank you for your interest in ShiftMed. Below you will find a Checklist of requirements for LPN/RNs that are interested in working through ShiftMed in New Hampshire.


Required Documents

First documents you will need to upload prior to completing your profile and uploading state specific requirements:

When signing up to work with ShiftMed in the app you will then be prompted to enter your

  • Proof of Identification (DL, Photo ID, Passport, etc.)

  • Social Security Number

    • You can upload multiple Professional Licenses and any Specialties if applicable.

Please Note: You must be professionally licensed to practice in this State.


Required Documents

Documents that must be signed in the ShiftMed App before you can start uploading credentials:

  • Training Manual - complete directly within the app using the tap to sign feature

  • Company Handbook - complete directly within the app using the tap to sign feature

  • TB Baseline Risk Agreement Form

  • Arbitration Agreement

Please note that specific facility credentials could be asked of you when attempting to claim a shift. These are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require. For more information on Facility Specific Credentials, CLICK HERE.

  • Annual Physical Exam: Completed within the last year.

  • Specialties: Some of our partner facilities frequently seek clinical support from healthcare professionals with specialized

  • COVID-19 Vaccine: Some partner facilities will require either a COVID-19 Vaccination (1 or 2 doses) or a Booster.

  • Varicella: Some of our partner facilities require this credential before claiming a shift.

  • TDAP: Some of our partner facilities require this credential before claiming a shift.

  • Flu Shot: Some of our partner facilities require this credential during flu season.

  • References: Some of our partner facilities require this credential before claiming a shift. Add your references directly in the app.

  • Specialties: Some of our partner facilities frequently seek clinical support from healthcare professionals with specialized skills. If you're certified in a specialty area of practice, you want this credential on file with us.

  • Skills Self Evaluation: Complete directly within in the app.

  • N95 Mask Fit: Some of our partner facilities may require this credential before claiming a shift.


Items that ShiftMed will Verify

  • Active New Hampshire LPN/RN Professional License

  • Clear record on the New Hampshire Abuse Check Registry (BEAS Registry Check Results)

  • Clear record on the Office of Inspector General (OIG) Exclusion Check

  • Clear record on the System for Award Management (SAM) Check
