What credentials do I need to work as a Pharmacist or Pharm Tech in Michigan?

Thank you for your interest in ShiftMed. Here is a Checklist of requirements for Pharmacists or Pharm Techs that are interested in working through ShiftMed in Michigan.


Required Documents

First documents you will need to upload prior to completing your profile and uploading state specific requirements:

When signing up to work with ShiftMed in the app you will then be prompted to enter your

  • Proof of Identification (DL, Photo ID, Passport, etc.)

  • Social Security Number

  • Professional License (1+ years of experience for Pharmacists / 6+ months of experience for Pharm Techs)

    • You can upload multiple Professional Licenses and any Specialties if applicable.

Please Note: You must be professionally licensed to practice in this State.


Required Forms

Forms that must be signed in the ShiftMed App before you can start uploading credentials:

  • Training Manual: Complete directly within the app using the tap to sign feature

  • Company Handbook: Complete directly within the app using the tap to sign feature

  • Arbitration Agreementt

  • Complete MI Background Check Consent Form: Complete directly within the app using the tab to sign feature

  • NPI Entry Form (Pharm Only)


Required Credentials

Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:

  • National Provider Identifier (NPI) - (Pharm Only)

  • Immunization Certification (Pharm Only)

Please note that specific facility credentials could be asked of you when attempting to claim a shift. These are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require. For more information on Facility Specific Credentials, CLICK HERE.

  • COVID-19 Vaccine: Some of our partner facilities require either a COVID-19 Vaccination (1-2 doses or booster).

  • Annual Physical Exam: Some of our partner facilities may require this credential before claiming a shift. Physicals are valid for 1 year.

  • License Certificate: The Professional License Certificate credential in the app is used to contain a document that a state’s licensing body issues after a healthcare professional has met the state requirements and passed a specific licensing exam for a particular professional license type.

  • 1-step PPD, 2-step PPD or TB Blood Test: PPD must be completed within the last year. Chest X-Ray must be completed within the last 5 years.

  • MMR: Some of our partner facilities require this credential before claiming a shift.

  • Varicella: Some of our partner facilities require this credential before claiming a shift.

  • References: Some of our partner facilities require this credential before claiming a shift. Add your references directly in the app.

  • Flu Shot: Some of our partner facilities require this credential during flu season.
