ShiftMed Requirements
To begin onboarding through ShiftMed, all healthcare professionals will need to submit and complete the following within the ShiftMed app:
Proof of Identification (DL, Photo ID, Passport, etc.)
Social Security Number
Professional License: (6+ months experience)
You can upload multiple Professional Licenses and any Specialties, if applicable.
Training Manual: Complete directly within the app using the tap to sign feature.
Company Handbook: Complete directly within the app using the tap to sign feature.
Arbitration Agreement
Submit Work History (this is like uploading a resume)
Credentialing Requirements
Candidates must complete the following credentialing expectations:

Certifications
CPR/BLS Certification
American Heart Association (AHA)
Health Requirements
Physical Exam (within the last year)
TB Screening:
QuantiFERON or T-Spot blood test within the last year, or
Two-Step PPD within the last year, or
Chest X-Ray (if applicable)
Flu Vaccine or Declination
COVID-19 Documentation (based on state or facility requirements)
MMR
Varicella
Hepatitis B
Tdap (if required)
Documentation
12-Hour Attestation Form
Abuse & Neglect Form
Dementia Training Form
Immunization and health records
Experience Requirements (Verified Specialties)
Experience requirements vary by facility and role.
Specialty experience may be required for certain units or assignments.

Orientation Details
Facility-specific orientation is required before the first shift.
Additional training modules may be assigned based on the facility or specialty.

1 buddy shift on the floor

Badge creation and system access setup (if applicable)
Introduction to charting and timekeeping processes
Additional education based on patient population or specialty area
ShiftMed Attendance Policy & Expectations
Clocking in and out:
You are required to clock in and out for every shift using the ShiftMed app.

Missed Punches:
If you are unable to use the app, you must promptly request a missed punch form from the Care Team to correct your time.
Cancelling a Shift:
If you need to cancel a shift, you must report it in the ShiftMed app.
Attendance Expectations:
While ShiftMed operates as an “on-demand” or “PRN” platform and may categorize attendance events - such as call-outs, cancellations, and unscheduled shifts - using a weighted system (see more details here), our hospital partners maintain high attendance expectations and may consider any change to a claimed shift to be an attendance violation.
Failure to adhere to these procedures, or patterns of excessive lateness or cancellations, may impact your future shift opportunities at all Autumn Lake Healthcare facilities.
