Using QR Code for Clock In/Out

We’re excited for you to work at an Autumn Lake Facility that is using Autumn Track/Touch Point technology for the first time, so let’s set you up for success! Before your arrival, get ready to clock in on the app and at the facility’s kiosk. This is to ensure that you get paid on time. Here’s what you need to do:

Clock In with QR Code:

  • Locate the facility's time clock to clock in.

  • Tap on "Clock In Details" on the ShiftMed app. This will share a QR code with you.

  • Scan the QR code at the facility's time clock and then select "Record Clock In".

  • Read the reminder screen for clocking out and then select "Got it"

Don’t forget to clock out when you’ve completed your shift.

Clock Out with QR Code:

  • Locate the facility's time clock to clock in.

  • Tap on "Clock Out Details" on the ShiftMed app. This will share a QR code with you.

  • Scan the QR code at the facility's time clock and then select "Record Clock Out"

Please note that 30 minutes for a meal break will be automatically deducted for every 5 hours worked. If you do not take a meal break please submit a Missed Punch Form via JotForm so that we can update your time sheet.

What if the QR Code fails to scan at the kiosk?

The QR code is a streamlined alternative to the existing manual process of checking in and out. If the QR code has any issues scanning at the kiosk, simply default to the original steps in clock in with your phone number, designating you are working for ShiftMed, and declare your Job Function (RN, CNA, etc.) when prompted by the kiosk. Do not contact the nursing supervisor for any additional support as needed to assist with a successful check in and out.

What if I forgot to scan in at the kiosk? Or can either QR code (in-app clock in versus clock out) be used at any time at the kiosk?

The QR code provided by the app during clock in must only be used to scan to check in at the kiosk. The QR code provided by app during clock out must only be used for check out. If any circumstances at the facility makes this difficult or you think you scanned in/out with the wrong code, contact the nursing supervisor for support. You may need to default to a manual check in/out. The QR codes are designed to inform Autumn Track when a scan equates to a clock in and to a clock out.

What if I did not take a 30 minute meal break?

A 30 minute meal break is automatically deducted for every 5 hours worked. If you do not take a meal break, please submit a Missed Punch Form via JotForm so that we can update your time sheet. When filling out the JotForm in the Break Duration field be sure to select "no break taken" and under Reason for missed punch select "Break Request". In the Detailed Reason for Missed Punch be sure include the reason that you did not take a meal break.
