Thank you for your interest in ShiftMed. Here is a Checklist of requirements for CNAs that are interested in working through ShiftMed in Georgia.
Please NOTE that you must have a Professional License through the state of Georgia.
Required Credentials:
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
Physical (completed within the last year)
ID
I-9
COVID-19 Vaccine - All HCPs must have either a COVID-19 Vaccination or must complete a Medical or Religious Exemption form in the app.
Training Manual - complete directly within the app using the tap to sign feature
Company Handbook - complete directly within the app using the tap to sign feature

Please note that these are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require:
COVID-19 Test - upload your most current negative COVID-19 test result (Please note that partner facilities with the clipboard symbol in-front of their name in the app - may have COVID-19 testing at their facility)
Health Insurance Card - to have on file for the COVID-19 Vaccine
Get 2 References - there are a few partner facilities that require verified references before claiming a shift - please enter mobile numbers for your references
Flu Shot - there are a few partner facilities in the area that require a flu shot during flu season before claiming a shift
Drug Screen - about 75% of our partners in this area require a drug screen and you will notified when you try to claim a shift if the facility requires one. 2-step PPD, 1-step PPD or TB Blood Test (completed within the last year) or Chest X-Ray (completed within the last 5 years) **You will have more facility options if you have a 2-step PPD vs a 1-step PPD.
Forms To Complete:
Complete GA Background Check Authorization - complete directly within the app using the tab to sign feature
Items that ShiftMed will Verify:
Fingerprint based background check through the Georgia Department of Community Health (GCHEXS)
Active Professional Georgia CNA License
Clear record on the Georgia Sexual Offender Registry
Clear record on the Office of Inspector General (OIG) Exclusion Check
Upload your credentials in the ShiftMed app.
