Thank you for your interest in ShiftMed. Here is a Checklist of requirements for LPNs or RNs that are interested in working through ShiftMed in this area.
Please NOTE that you must have a Professional License through the state of Louisiana.
Required Credentials:
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
Louisiana Professional License
ID
SSN entered in app
I-9
Resume - recommended but not required
Training Manual - complete directly within the app using the tap to sign feature
Company Handbook - complete directly within the app using the tap to sign feature
Additional Credentials:
Please note that these are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require:
COVID-19 Screening - upload your most current negative COVID-19 test result (Please note that partner facilities with the clipboard symbol in-front of their name in the app - may have COVID-19 testing at their facility)
Health Insurance Card - to have on file for the COVID-19 Vaccine
Annual Physical
Get 2 References - there are a few partner facilities that require verified references before claiming a shift - please enter mobile numbers for your references
MMR - some of our partner facilities require this credential before claiming a shift
Flu Shot - some of our partner facilities in the area do require a flu shot during flu season
Varicella - some of our partner facilities require this credential before claiming a shift
Forms To Complete:
Completed CheckR consent to start a Criminal Background Check - complete directly within the app using the tab to sign feature once you have uploaded your SSN, 1-9, Professional License and ID.
Items that ShiftMed will Verify:
Background Check completed through Checkr
Active Professional Louisiana LPN or RN License
Clear record on the National Sex Offender Registry Check
Clear record on the Louisiana State Adverse Actions List Search
Clear record on the Office of Inspector General (OIG) Exclusion Check
Please NOTE: To claim shifts in your area, you may be prompted to submit the following additional documents in the app. Please visit the My Credentials tab to upload this information. The sooner you add, the faster you can claim shifts. TB Test, Physical, COVID-19 Vaccine, CPR
