Required Documents
First documents you will need to upload prior to completing your profile and uploading state specific requirements:
When signing up to work with ShiftMed in the app you will then be prompted to enter your
- Proof of Identification (DL, Photo ID, Passport, etc.) 
- Social Security Number 
- Professional License: You can upload multiple Professional Licenses and any Specialties if applicable. 
Please Note: You must be professionally licensed to practice in this State.
Required Forms
Documents that must be signed in the ShiftMed App before you can start uploading credentials:
- Training Manual: Complete directly within the app using the tap to sign feature 
- Company Handbook: Complete directly within the app using the tap to sign feature 
- TB Baseline Risk Agreement Form 
- Arbitration Agreement 
- AART Certification 
Required Credentials
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
- Two (2) References: Please provide mobile numbers for your references 
Unlock Credentials
Please note that specific facility credentials could be asked of you when attempting to claim a shift. These are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require. For more information on Facility Specific Credentials, CLICK HERE.
- 1-step PPD, 2-step PPD or TB Blood Test: PPD must be completed within the last year. Chest X-Ray must be completed within the last 5 years 
- Annual Physical Exam: Completed within the last year. 
- COVID-19 Vaccine: Some partner facilities will require either a COVID-19 Vaccination (1 or 2 doses) or a Booster. 
- MMR: Some of our partner facilities require this credential before claiming a shift. 
- Varicella: Some of our partner facilities require this credential before claiming a shift. 
- TDAP: Some of our partner facilities require this credential before claiming a shift. 
- Flu Shot: Some of our partner facilities require this credential during flu season. 
- Resume: Some of our partner facilities may require this credential before claiming a shift. 
- CPR: Some partner facilities may require CPR. Depending on your role and the shift you are working, a specific certification may be needed. Be sure to confirm Facility Specific Credentials. 
Items that ShiftMed will Verify
- Active North Carolina Rad Tech Professional License 
- Depending on your NC residency the following Background Check will be conducted: - Name-Based Criminal Record Background Check: For applicants that have been a resident of NC for 5+ years - a name-based Criminal Record Background Check will be completed through the NC Department of Health and Human Services. 
- Fingerprint-Based Background Check: For applicants that have not been a resident of NC for at least 5 years - a Fingerprint-Based Background Check will be required through the NC Department of Health and Human Services. 
 
 
- Clear record on the Office of Inspector General (OIG) Exclusion Check 
- Clear record on the System for Award Management (SAM) Exclusion Check
