Thank you for your interest in ShiftMed. Here is a Checklist of requirements for CNAs, that are interested in working through ShiftMed in the state of Pennsylvania.
Required Documents
First documents you will need to upload prior to completing your profile and uploading state specific requirements:
When signing up to work with ShiftMed in the app you will then be prompted to enter your
Proof of Identification (DL, Photo ID, Passport, etc.)
Social Security Number
You can upload multiple Professional Licenses and any Specialties if applicable.
Please Note: You must be professionally licensed to practice in this State.

Training Manual: Complete directly within the app using the tap to sign feature
Company Handbook: Complete directly within the app using the tap to sign feature

Required Credentials
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
Annual Physical Exam: Completed within the last year.
2-step PPD or TB Blood Test: PPD must be completed within the last year. Chest X-Ray must be completed within the last 5 years

Please note that specific facility credentials could be asked of you when attempting to claim a shift. These are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require. For more information on Facility Specific Credentials, CLICK HERE.
COVID-19 Vaccine: Some partner facilities will require either a COVID-19 Vaccination (1 or 2 doses) or a Booster.
MMR: Some of our partner facilities require this credential before claiming a shift.
Varicella: Some of our partner facilities require this credential before claiming a shift.
TDAP: Some of our partner facilities require this credential before claiming a shift.
Flu Shot: Some of our partner facilities require this credential during flu season.
Resume: Some of our partner facilities may require this credential before claiming a shift.
References: Some of our partner facilities require this credential before claiming a shift. Add your references directly in the app.
Specialties: Some of our partner facilities frequently seek clinical support from healthcare professionals with specialized skills. If you're certified in a specialty area of practice, you want this credential on file with us.
Skills Self Evaluation: Complete directly within in the app.
N95 Mask Fit: Some of our partner facilities may require this credential before claiming a shift.
CPR: Some of our partner facilities may require this credential before claiming a shift
PA Department of Human Services Fingerprinting: Some partner facilities may require this credential before claiming a shift. If required, you will receive an email with further instructions on how to complete this credential.
PA Child Abuse History Certification: Some partner facilities may require this credential before claiming a shift. If required, you will receive an email with further instructions on how to complete this credential.
Items that ShiftMed will Verify
Active PA CNA Professional License
CheckR Background Check + PA PATCH Criminal Background Check (completed within the last year)
Proof of Residency: See special requirements below for details
Clear record on the Office of Inspector General (OIG) Exclusion Check
Clear record on the System for Award Management (SAM) Exclusion Check
Special Requirements for Proof of Residency
If you have been a resident of PA for over 2+ years we can accept a PA issued ID that was issued at least 2 years ago.
If your ID was issued less than 2 years ago, please provide TWO of the following documents to verify your proof of residency:
Expired PA ID
Utility Bill
Tax Statements
Credit or Bank Statement
Mortgage Statement or Rental Lease
Note: The documents must contain your full legal name, PA address and a date that is from 2+ years ago.
If you are NOT a resident of PA or have not been a resident of PA for the past 2-years, an FBI Background through the Department of Aging is REQUIRED.
