Taxes: Everything You Need to Know

Learn how taxes work as a ShiftMed W-2 employee, where to access and update your tax forms, and important information about W-4 changes, tax withholding, and claiming tax exemptions.

ShiftMed will mail your W-2 to the address we have on file, so it's important to make sure what we have is current. You can also access your W-2 Form in Paylocity.

You're a W-2 Employee

As a ShiftMed healthcare professional, you are a W-2 employee. This means ShiftMed withholds required taxes from your paycheck and reports your earnings to the IRS.


Your W-2 Form

ShiftMed will mail your W-2 each year to the address we have on file, so please keep your address up to date.

You can also access your W-2 in Paylocity.

Your W-2 shows:

  • How much you earned during the year

  • How much was withheld for taxes

You'll need your W-2 to file your annual tax return.


Understanding Your Pay

The pay shown in the ShiftMed app is your gross pay (before taxes and deductions).

Your actual paycheck may be lower because taxes and other deductions are taken out before payment.


Taxes Withheld from Your Paycheck

Depending on your situation and state of residence, your paycheck may include:

  • Federal income tax

  • State income tax (if applicable)

  • Social Security tax

  • Medicare tax


Updating Your W-4 Form in ShiftMed App

Your W-4 tells ShiftMed how much federal income tax to withhold from your paycheck.

You can update your W-4 at any time in the Banking & Taxes section of the ShiftMed app.

Important: Do not update your W-4 in Paylocity. Changes made in Paylocity will not sync to the ShiftMed app.

To update your W-4:

  1. Open the ShiftMed app and go to Banking & Taxes.

  2. Complete the verification process using the code sent to your phone.

  3. Submit your changes within the time allowed.

If you're unsure how to complete your W-4, please consult a tax professional or review the IRS W-4 instructions.


Claiming Tax Exemption ("BLOCKED" Status)

If you are claiming exemption from federal tax withholding:

  1. Download and complete a W-4 form.

  2. Sign the form.

Please allow:

  • 3–5 business days for processing

  • Up to two pay cycles for changes to take effect

Note: Tax exemptions expire annually. A new signed W-4 must be submitted each year to maintain your exemption status.


State Tax Forms

If your state requires state income tax withholding, you may submit a state tax form by emailing it to shiftmedpayroll@shiftmed.com.

Please allow 2–3 business weeks for updates to appear on your paycheck.
