Thank you for your interest in ShiftMed. Here is a Checklist of requirements for LPNs and RNs that are interested in working through ShiftMed in this area.
Please NOTE that you must have a Professional License through the state of New Mexico.
Required Credentials:
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
2-step PPD or TB Blood Test (completed within the last year) or Chest X-Ray (completed within the last 5 years)
ID
I-9
CPR
Resume - recommended but not required
Training Manual - complete directly within the app using the tap to sign feature
Company Handbook - complete directly within the app using the tap to sign feature
Additional Credentials:
Please note that additional facility specific credentials could be asked of you when attempting to claim a shift. These are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require.
COVID-19 Vaccine - All HCPs must have either a COVID-19 Vaccination or must complete a Medical or Religious Exemption form in the app.
COVID-19 Test - upload your most current negative COVID-19 test result (Please note that partner facilities with the clipboard symbol in-front of their name in the app - may have COVID-19 testing at their facility)
Health Insurance Card - to have on file for the COVID-19 Vaccine
Flu Shot - there are a few partner facilities in the area that require a flu shot during flu season before claiming a shift
Physical (completed within the last year)
Get 2 References - please enter mobile numbers for your references
TNCC (Trauma Nurse Core Care) - RNs Only
Forms To Complete:
Complete New Mexico Background Check Authorization - complete directly within the app using the tab to sign feature once you have uploaded your TB Test, and ID.
Items that ShiftMed will Verify:
Active Professional New Mexico LPN/RN License
Clear record on the Office of Inspector General (OIG) Exclusion Check
Upload your credentials in the ShiftMed app.
