What credentials do I need to work as an LPN or RN in Massachusetts?

Thank you for your interest in ShiftMed. Here is a Checklist of requirements for LPNs and RNs that are interested in working through ShiftMed in Massachusetts.


Required Documents:

First documents you will need to upload prior to completing your profile and uploading state specific requirements.

When signing up to work with ShiftMed in the app you will then be prompted to enter your:

  • Proof of Identification (DL, Photo ID, Passport, etc.)

  • Social Security Number

  • Professional License (6+ months experience)

    • You can upload multiple Professional Licenses and any Specialties, if applicable.

Please NOTE that you must have a Professional License through the state of Massachusetts.

Once uploaded you will be prompted to sign your documents and upload the required credentials for your state. Keep in mind some Facilities may have additional requirements aside from the state specifics. See below for more information on what your state requires.


Required Forms:

Forms that must be signed in the ShiftMed App before you can start uploading credentials:

  • Training Manual: Complete directly within the app using the tap to sign feature.

  • Company Handbook: Complete directly within the app using the tap to sign feature.

  • MA CORI Acknowledgement Form

  • Personal Information Release Acknowledgment

  • TB Baseline Risk Agreement Form


Required Credentials:

Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:

  • COVID-19 Vaccine: All HCPs must have either a COVID-19 Vaccination (1-2 doses or booster) or must complete a Medical or Religious Exemption form in the app.


Additional Credentials:

Please note that additional facility specific credentials could be asked of you when attempting to claim a shift. These are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require. For more information on Facility Specific Credentials, CLICK HERE.

  • License Certificate: The Professional License Certificate credential in the app is used to contain a document that a state’s licensing body issues after a healthcare professional has met the state requirements and passed a specific licensing exam for a particular professional license type.

  • 1-step PPD, 2-step PPD or TB Blood Test (completed within the last year) or Chest X-Ray (completed within the last 5 years)

    • Disclaimer: 1-step PPD is the minimum testing requirement, however specific facilities may have additional TB testing requirements at their discretion.

  • MMR: 2 dose vaccination series or titer.

  • Flu Shot: Some of our partner facilities in the area do require a flu shot during flu season.

  • Varicella: 2 dose vaccination series or titer.

  • Hepatitis B: 3 dose vaccination series or titer.

  • TDAP: Vaccine record (valid for 10 years) or titer.

  • Resume: Can be uploaded or completed directly in the app using our Resume Builder.

  • Specialties: Some of our partner facilities frequently seek clinical support from healthcare professionals with specialized skills. If you're certified in a specialty area of practice, you want this credential on file with us.

  • Skills Self Evaluation: Complete directly within in the app.

  • NIH Stroke Scale Course: The NIH Stroke Scale (NIHSS) is a critical component of acute stroke assessment and may be required for certain Specialties.

  • Crisis Prevention Training: This training is for primary caregivers, educators, and healthcare professionals who directly intervene in crisis situations, teaching staff de-escalation techniques as well as restrictive and nonrestrictive interventions. This is the most commonly required CPI certification type required by our facility partners.

  • Online Learning Certificate: We have partnered with some of our Acute Health Systems to provide learning modules for our RNs, CNAs, and RTs.


Items that ShiftMed will Verify:

  • Active Professional Massachusetts LPN or RN License

  • Clear record on the Office of Inspector General (OIG) Exclusion Check

  • Clear record on the System for Award Management (SAM) Exclusion Check
