Thank you for your interest in ShiftMed. Here is a Checklist of requirements for LPNs and RNs that are interested in working through ShiftMed in this area.
Please NOTE that you must have a Professional License through the state of Pennsylvania.
Required Credentials:
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
Physical (completed within the last year)
2-step PPD or TB Blood Test (completed within the last year) or Chest X-Ray (completed within the last 5 years)
ID
I-9
CPR
Resume - recommended but not required
COVID-19 Vaccine - All HCPs must have either a COVID-19 Vaccination or must complete a Medical or Religious Exemption form in the app.
Training Manual - complete directly within the app using the tap to sign feature
Company Handbook - complete directly within the app using the tap to sign feature
LPN or RN Job Description - complete directly within the app using the tap to sign feature
Additional Credentials:
Please note that these are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require:
COVID-19 Test - upload your most current negative COVID-19 test result (Please note that partner facilities with the clipboard symbol in-front of their name in the app - may have COVID-19 testing at their facility)
Health Insurance Card - to have on file for the COVID-19 Vaccine
Get 2 References - there are a few partner facilities that require verified references before claiming a shift - please enter mobile numbers for your references
License Certification - about 30% of our partner facilities require a copy of your professional license before claiming a shift
Flu Shot - about 50% of our partner facilities in the area does require a flu shot during flu season (effective 11/1/20) before claiming a shift
Drug Screen - the majority of our partners in Pittsburgh require a drug screen. You will notified when you try to claim a shift if the facility requires one. Forms To Complete:
Complete Checkr consent to start a Criminal Background Check (PA Patch Background Check) - complete directly within the app using the tab to sign feature once you have uploaded your Physical, TB Test and ID. - Under My Credentials scroll to Background Check - it will be listed as Missing - select - and then complete the consent form in the app
Items that ShiftMed will Verify:
PA PATCH Criminal Background Check (completed within the last year)
Active Professional PA LPN/RN License
Clear record on the Office of Inspector General (OIG) Exclusion Check
Special Requirements:
PA Residency - requires an FBI Background through the Department of Aging if either NOT a resident of PA or has not been a resident of PA for the passed 2-years - https://uenroll.identogo.com/ (Enter 1KG8RJ as the service code and later in the process when asked for a Facility ID you can use 14623.)
If you have been a resident of PA for over 2+ years we can accept a PA issued ID that was issued at least 2 years ago. If you do not have a PA issued ID that provides this proof you can supply two of the following documents: Utility Bill, Tax Statements, Credit or Bank Statement, Mortgage, Rental Lease. The documents must contain your full legal name, PA address and a date that is from 2+ years ago.
