Thank you for your interest in ShiftMed. Here is a Checklist of requirements for LPNs and RNs that are interested in working through ShiftMed in this area.
Please NOTE that you must have a Professional License through the state of Ohio.
Required Credentials:
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
Physical (completed within the last year)
2-step PPD, 1-step PPD or TB Blood Test (completed within the last year) or Chest X-Ray (completed within the last 5 years) **Disclaimer - a 1-step PPD is the minimum testing requirement, however specific facilities may have additional TB testing requirements at their discretion.

Background Check - if you have a prior BCI Ohio Background check or a receipt for search confirmation please submit to bg@shiftmed.com.
COVID-19 Vaccine - All HCPs must have either a COVID-19 Vaccination or must complete a Medical or Religious Exemption form in the app.
Training Manual - complete directly within the app using the tap to sign feature
Company Handbook - complete directly within the app using the tap to sign feature

Please note that these are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require:
COVID-19 Test - upload your most current negative COVID-19 test result that was collected within the last 2-weeks (Please note that partner facilities with the clipboard symbol in-front of their name in the app - may have COVID-19 testing at their facility)
Health Insurance Card - to have on file for the COVID-19 Vaccine
Get 2 References - there are a few partner facilities that require verified references before claiming a shift - please enter mobile numbers for your references
Flu Shot - there are a few partner facilities in the area does require a flu shot during flu season before claiming a shift
Drug Screen - this is required at some facilities and you will notified when you try to claim a shift if the facility requires one. If you would like a drug screen to be scheduled, please reach out to us in chat by going into the ShiftMed App > Menu > Help Center > Help Articles & Chat > Messages > Send Us A Message!
Items that ShiftMed will Verify:
If a prior BCI Criminal Background Check is submitted this will be verified by ShiftMed.
Active Professional Ohio LPN/RN License
Clear record on the Office of Inspector General (OIG) Exclusion Check
Special Requirements:
Ohio Residency - requires an FBI Background Check if the caregiver is NOT a resident of Ohio or has not been a resident of Ohio for the passed 5-years
