Thank you for your interest in ShiftMed. Here is a Checklist of requirements for LPN and RNs that are interested in working through ShiftMed in this area.
Please NOTE that you must have a Professional License through the state of Michigan.
Required Credentials:
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
Physical (completed within the last year)
1-step PPD, 2-step PPD or TB Blood Test (completed within the last year) or Chest X-Ray (completed within the last 5 years) **Disclaimer - a 1-step PPD is the minimum testing requirement, however specific facilities may have additional TB testing requirements at their discretion.

Resume - recommended but not required
Background Check - If you do not have a current Michigan Fingerprint Background check, we will schedule you to get one completed.
COVID-19 Vaccine - All HCPs must have either a COVID-19 Vaccination or must complete a Medical or Religious Exemption form in the app.
Training Manual - complete directly within the app using the tap to sign feature
Company Handbook - complete directly within the app using the tap to sign feature

Please note that these are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require:
COVID-19 Test - upload your most current negative COVID-19 test result that was collected within the last 2-weeks (Please note that partner facilities with the clipboard symbol in-front of their name in the app - may have COVID-19 testing at their facility)
Health Insurance Card - to have on file for the COVID-19 Vaccine
Get 2 References - there are a few partner facilities that require verified references before claiming a shift - please enter mobile numbers for your references
Flu Shot - there are a few partner facilities in the area that require a flu shot during flu season before claiming a shift
Drug Screen - this is required at some facilities and you will notified when you try to claim a shift if the facility requires one. Complete MI Background Check Consent Form - complete directly within the app using the tab to sign feature
Items that ShiftMed will Verify:
Fingerprint-based Background Check through the Michigan Department of Licensing and Regulatory Affairs (LARA)
Active Professional Michigan LPN or RN License
Clear record on the Office of Inspector General (OIG) Exclusion Check
Upload your credentials in the ShiftMed app.
