Thank you for your interest in ShiftMed. Here is a Checklist of requirements for LPNs and RNs that are interested in working through ShiftMed in this area.
Please NOTE that you must have a Professional License through the state of Florida - or have a Compact State Professional License.
Required Credentials:
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
Physical (completed within the last year)
ID
I-9
CPR
Get 2 References - please enter their mobile number into the app
Resume - recommended but not required
COVID-19 Vaccine - All HCPs must have either a COVID-19 Vaccination or must complete a Medical or Religious Exemption form in the app.
Training Manual - complete directly within the app using the tap to sign feature
Company Handbook - complete directly within the app using the tap to sign feature
Additional Credentials:
Please note that these are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require:
COVID-19 Test - upload your most current negative COVID-19 test result (Please note that partner facilities with the clipboard symbol in-front of their name in the app - may have COVID-19 testing at their facility)
Health Insurance Card - to have on file for the COVID-19 Vaccine
Flu Shot - there are a few partner facilities in the area that require a flu shot during flu season before claiming a shift
2-step PPD, 1-step PPD or TB Blood Test (completed within the last year) or Chest X-Ray (completed within the last 5 years) **You will have more facility options if you have a 2-step PPD vs a 1-step PPD.
IV Therapy Certificate - LPNs Only - we can accept a FL - 30 hour training certificate or FL - 20 hour training certificate
Forms To Complete:
Complete Florida Background Check Authorization - complete directly within the app using the tab to sign feature
Items that ShiftMed will Verify:
Level 2 - Fingerprint based background check through the Agency for Health Care Administration (AHCA)
Active Professional Florida LPN/RN License
Clear record on the Office of Inspector General (OIG) Exclusion Check
Upload your credentials in the ShiftMed app.
