TN - Tennessee LPN & RN Credentialing Requirements

What credentials do I need to work as an LPN or RN in Tennessee?

Thank you for your interest in ShiftMed. Here is a Checklist of requirements for LPNs or RNs that are interested in working through ShiftMed in Tennessee.


Required Documents

First documents you will need to upload prior to completing your profile and uploading state specific requirements:

When signing up to work with ShiftMed in the app you will then be prompted to enter your

  • Proof of Identification (DL, Photo ID, Passport, etc.)

  • Social Security Number

  • Professional License (LPN/RN - with 6+ months experience)

    • You can upload multiple Professional Licenses and any Specialties if applicable.

Please Note: You must be professionally licensed to practice in Tennessee


Required Forms

Documents that must be signed in the ShiftMed App before you can start uploading credentials:

  • Training Manual: Complete directly within the app using the tap to sign feature

  • Company Handbook: Complete directly within the app using the tap to sign feature

  • TB Baseline Risk Agreement Form

  • Arbitration Agreement

Please note that specific facility credentials could be asked of you when attempting to claim a shift. These are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require. For more information on Facility Specific Credentials, CLICK HERE.

  • Annual Physical Exam: Completed within the last year.

  • COVID-19 Vaccine: Some partner facilities will require either a COVID-19 Vaccination (1 or 2 doses) or a Booster.

  • MMR: Some of our partner facilities require this credential before claiming a shift.

  • Varicella: Some of our partner facilities require this credential before claiming a shift.

  • TDAP: Some of our partner facilities require this credential before claiming a shift.

  • Flu Shot: Some of our partner facilities require this credential during flu season.

  • References: Some of our partner facilities require this credential before claiming a shift. Add your references directly in the app.

  • Specialties: Some of our partner facilities frequently seek clinical support from healthcare professionals with specialized skills. If you're certified in a specialty area of practice, you want this credential on file with us.

  • Skills Self Evaluation: Complete directly within in the app.

  • N95 Mask Fit: Some of our partner facilities may require this credential before claiming a shift.

  • Clear record on the Tennessee Abuse Check Registry

  • Clear record on the Tennessee Sex Offender Check Registry

  • Clear record on the Office of Inspector General (OIG) Exclusion Check

  • Clear record on the System for Award Management (SAM) Exclusion Check
