What credentials do I need to work as an LPN or RN in Tennessee?

Required Documents

First documents you will need to upload prior to completing your profile and uploading state specific requirements:

When signing up to work with ShiftMed in the app you will then be prompted to enter your

  • Proof of Identification (DL, Photo ID, Passport, etc.)

  • Social Security Number

  • Professional License (LPN/RN - with 6+ months experience)

    • You can upload multiple Professional Licenses and any Specialties if applicable.

Please Note: You must be professionally licensed to practice in Tennessee


Required Forms

  • Training Manual: Complete directly within the app using the tap to sign feature

  • Company Handbook: Complete directly within the app using the tap to sign feature

  • TB Baseline Risk Agreement Form

  • Arbitration Agreement

Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:

Please note that specific facility credentials could be asked of you when attempting to claim a shift. These are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require. For more information on Facility Specific Credentials, CLICK HERE.

  • Criminal Background Check through Checkr: some partner facilities may require a background check.

  • 1-step PPD, 2-step PPD or TB Blood Test: Some partner facilities may require this credential.

    • PPD typically good for 1 year

    • Chest xray typically good for 5 years, but some facilities may require yearly.

  • CPR: Some partner facilities may require CPR. Depending on your role and the shift you are working, a specific certification may be needed. Be sure to confirm Facility Specific Credentials.

  • Annual Physical Exam: Some partner facilities may require this credential.

  • COVID-19 Vaccine: Some partner facilities may require this credential.

  • MMR: Some partner facilities may require this immunization.

    • 2 doses vaccination series or titer

  • Flu Shot: Some of our partner facilities in the area do require a flu shot during flu season.

  • Varicella: Some partner facilities may require this immunization.

    • 2 doses vaccination series or titer

  • Resume: Can be uploaded or completed directly in the app using our Resume Builder.

  • References: Enter their mobile number into the app.

  • Drug Screening: Some partner facilities may require this credential.

  • Active Tennessee LPN or RN Professional License

  • Clear record on the Tennessee Abuse Check Registry

  • Clear record on the Tennessee Sex Offender Check Registry

  • Clear record on the Office of Inspector General (OIG) Exclusion Check

  • Clear record on the System for Award Management (SAM) Exclusion Check
