Thank you for your interest in ShiftMed. Here is a Checklist of requirements for CNAs that are interested in working through ShiftMed in this area.
Please NOTE that you must have a Professional License through the state of Texas.
Required Credentials:
Documents that must be uploaded into the ShiftMed App before you can start claiming shifts:
1-step PPD, 2-step PPD, or TB Blood Test (completed within the last year) or Chest X-Ray (completed within the last 5 years)
ID
I-9
COVID-19 Vaccine - All HCPs must have either a COVID-19 Vaccination or must complete a Medical or Religious Exemption form in the app.
Training Manual - complete directly within the app using the tap to sign feature
Company Handbook - complete directly within the app using the tap to sign feature

Please note that these are NOT required for you to be hired by ShiftMed. These are special requirements that some of our partner facilities require:
COVID-19 Test - upload your most current negative COVID-19 test result that was collected within the last 2-weeks (Please note that partner facilities with the clipboard symbol in-front of their name in the app - may have COVID-19 testing at their facility)
Health Insurance Card - to have on file for the COVID-19 Vaccine
COVID-19 Vaccine - if you receive the Pfizer, Moderna, Johnson & Johnson Get 2 References - there are a few partner facilities that require verified references before claiming a shift - please enter mobile numbers for your references
Flu Shot - there are a few partner facilities in the area that require a flu shot during flu season before claiming a shift
Physical (completed within the last year)
Forms To Complete:
Complete CheckR consent to start a Criminal Background Check - complete directly within the app using the tab to sign feature once you have uploaded your TB Test, and ID.
Items that ShiftMed will Verify:
Criminal Background Check through CheckR
Active Professional Texas CNA License
Clear record on the Office of Inspector General (OIG) Exclusion Check
Valid status on the Texas Employability Status Check Search
Upload your credentials in the ShiftMed app.
